We have a team of Clinical Pharmacists, Pharmacy Technicians and Care Coordinators working across the practices in the PCN providing a Pharmacy Service.
The team members have vast experience from different settings, including community pharmacy, hospital and ICB (Integrated Care Board). This brings a diverse skill mix to the team and supports us in providing a service to practices which includes:
Medication Reviews – assessing their safety and effectiveness.
Structured Medication Reviews – holistic in-depth reviews of medication
Medication queries
Actioning medication changes after a hospital admission or an outpatient clinic visit
Medication reconciliation for new patients to the surgery
Pharmacists complete a 4-year Masters of Pharmacy degree in order to become medication experts. On qualifying, they undertake a year-long foundation training programme and sit the General Pharmaceutical Council registration exam in order to register. Once registered, they are expected to continue with post-qualification learning and development. This includes working towards an Independent Prescriber qualification.
